Prior to your first visit you must fill out a Health History Questionnaire. You may use the following link to download and print the Health History Questionnaire, or when scheduling your appointment you may request one be mailed to you. Either way, please remember to bring your Health History Questionnaire filled out as completely and accurately as possible to your first appointment.
